Request certification of full-time student status for students registering less than 12 (MS) or 9 credits (PhD). Must be working on project/dissertation/preparing for the comprehensive exam. See Graduate School Forms for Students.
For BS and BS/MS students who need force registration in PHC 498 Research, PHC 499 Independent Study, PHC 506 Biometry, or PHC 616 Research. Seek academic advisement and approvals before submitting.
For students registering for a course outside of their academic career (ex: a graduate student wishing to take a non-PHC graduate course).
For postdocs wishing to audit a course. To audit a course, ALL students must officially register and submit a written request to the instructor by the fourth week of class. See the Graduate School N Grade Policy.
For transferring non-UB PHC courses into the PhD program. Students are permitted to transfer up to 36 credit hours, subject to approval of the Director of Graduate Studies (DGS).
Students must register for at least one credit EACH semester until their degree is conferred. For more information on the following forms, see Graduate School Policy.
For students with a break in attendance. Must seek approval before submitting to the Registrar's Office.
For students with extenuating circumstances looking to convert all semester grades to 'W's.
PhD students submit this form each year by February 15. It is used to provide updates on your professional performance. Your faculty advisor will also submit an update.
PhD students are required to schedule a committee meeting within one month of presenting a Department Seminar. At least three formal committee meetings are required: One for the oral proposal defense and two following departmental seminars. Meetings should occur annually after the Preliminary Exam.
Submit by May 1/December 1 to indicate your plans to take the preliminary exam.
