Preceptors
You may either submit your application electronically using the "Submit by Email" button or mail a copy to the address at the bottom of the form by using the "Print Form" button. Adobe Reader 8 is required to fill out the form. Submission by email is preferred to submission by mail. Mac users, please make sure that Adobe Acrobat 8 is your default PDF viewer in Safari.
- After clicking "Submit by Email", follow the on-screen instructions, selecting the appropriate option that matches your e-mail preferences.
- If using Outlook, an e-mail draft will be saved to your Outbox. Open your Outbox, double click the e-mail named "Form Returned: APPE_Preceptor_Info_Form_pub.pdf" and click send.
