Criminal Background Checks Policy

All accepted applicants and enrolled students in the PharmD program (P1 through P4) must submit and complete a criminal background check (CBC). Failure to request a CBC review in a timely manner is grounds for UB SoPPS to withdraw the offer of admission for new P1 students; P2-P4 students who refuse to obtain a CBC may be withheld from any experiential rotation (i.e., introductory or advance) and may be subsequently dismissed from the PharmD program. Transportation Policy

UB SoPPS requires all doctor of pharmacy program (PharmD) students to have their own transportation. The school cannot guarantee that students will be placed in clinical rotation sites within UB transportation. Therefore, as a student, you will be responsible for providing your own transportation throughout all four professional years.

 

Health Background Form

All students must complete and return a Health Background Form no later than February 15 or within 10 days of their Admission Acceptance Confirmation email. This form includes a full health and immunization history. After the P1 year, students will be required to update their shots on a yearly basis and submit the Annual Health Update Form.
