2. Log onto the system with your user ID and password
3. Select "Applications" at the top of the page. Click on the
letter "P"
4. Scroll down to "Prescription Monitoring Program Registry"
5. Click to open the program
6. Enter patient information
7. Attest to the guidelines
8. Review the Frequently Asked Questions within the
application for further information
