Attention Pharmacists
Diabetes Care Certificate Program
The University at Buffalo School of Pharmacy has developed a Diabetes Care Certificate Program. This course consists of approximately 30-40 hours of didactic and experiential training and doumentation of patient interventions. A
strong commitment to the care of patients with diabetes is a prerequisite.
The goals of this course are:
- to develop a comprehensive didactic and clinical training program in diabetes management for practicing pharmacists
- to provide pharmacists with the tools necessary to effectively deliver pharmaceutical care to the patient with diabetes
- to establish a network of ?diabetes care centers? located in pharmacies throughout Western New York
- to establish a network of pharmacists who can serve as a referral source for physicians, nurse educators and other health professionals
- to document positive patient care outcomes as a result of the intervention of pharmacists on behalf of patients with diabetes
- to earn third party reimbursement for diabetes education coordinated by pharmacists
