Important Policies

Please carefully read the following UB School of Pharmacy and Pharmaceutical Sciences (UB SoPPS) policies:


Transportation Policy


UB SoPPS requires all doctor of pharmacy program (PharmD) students to have their own transportation. The school cannot guarantee that students will be placed in clinical rotation sites within UB transportation. Therefore, as a student, you will be responsible for providing your own transportation throughout all four professional years.

Health Background Form


All new transfer students must complete and return a Health Background Form, which includes a full health and immunization history. All continuing UB students must complete and return an Annual Health Update Form. After the P1 year, students will be required to update their shots on a yearly basis and submit the Annual Health Update Form.

A completed Health Background Form must be on file with Health Services on which the following information must be documented:

  • MMR vaccination with 2 Measles shots or immunity to Measles, Mumps, and Rubella
  • Tetanus Vaccination within the past 10 years
  • PPD (Tuberculin Skin Test). All students with no prior history of positive PPD should have a Mantoux Tuberculin Skin Test within 12 months prior to starting their program. Note: Tine Test cannot be substituted for PPD; Prior BCG or pregnancy is not an exemption; Chest X-Ray cannot be substituted for PPD; Chest X-Ray is required if PPD is positive or if student has a prior history of a positive PPD.
  • Students must be fully immunized against Hepatitis B by the end of the first year. This three-injection series must be started within 1 month of the first semester.
  • A history of Varicella Disease, Vaccine, or Immunity: The CDC and ACHA recommend the vaccine series for those students who have not had the chicken pox. For adults, this is a two shot vaccine series.
  • Meningococcal (Meningitis) Vaccine: The CDC and ACHA recommend that college students consider this vaccine, which offers some protection against bacterial meningitis. College students, particularly students who live in residence halls, are at increased risk for meningitis.

You must submit the Health Background Form or Annual Health Update Form by September 1st.  This must be sent to: University at Buffalo Student Health Services, Michael Hall, 3435 Main Street, Buffalo, New York 14214-8003. Also, please send a copy of your completed form to: University at Buffalo School of Pharmacy and Pharmaceutical Sciences, Attn: Dr. Peter Brody, Director of Experiential Education, 373 Cooke Hall, Buffalo, New York 14260. It is highly recommended that students also keep a copy of this form for their records.

Criminal Background Checks Policy


Policy
All accepted applicants and enrolled students in the PharmD program (P1 through P4) must submit and complete a CBC from a vendor approved by UB SoPPS (Verified Credentials - www.verifiedcredentials.com) at the student's expense. Students who refuse to submit to the CBC may be dismissed from the program. An accepted applicant must initiate the CBC review within 15 days of receiving the notice of this policy. A notice will be included in the "Admissions Acceptance Confirmation" letter, which will be mailed once payment and acceptance forms are received. Failure to request a CBC review in a timely manner is grounds for UB SoPPS to withdraw the offer of admission. Students currently enrolled in the PharmD program must request from the School's vendor a CBC during the month of February in the spring semester. Students who refuse to obtain a CBC may be withheld from any experiential rotation (i.e., introductory or advance) and may be subsequently dismissed from the PharmD program.
